CASE MANAGER
Reports to: Program Director
Responsibilities: Welcomes clients in a hospitable manner, in order to establish a positive connection. Provides comprehensive case management services to clients through pychosocial assessment, intervention, referral and follow-up. Participates in ACT team Model of Care (i.e. daily staffing, street and home visits). Provides clients with case management services. Identifies and coordinates Camillus House and community services for clients.
Documents all client and referral encounters in the clinical records. Advocates for clients within Camillus House and the community.
Works effectively and professionally with Camillus House staff and community partners.
Participates in weekly case conferences and supervisory sessions. Other duties as assigned.
Requirements: Committed to the mission of Camillus House in both knowledge and practice.
Demonstrated ability to read and write English at the 12th grade level. Bilingual(English/Spanish), additional languages a plus (i.e. Creole). Valid FL driver’s license.
Demonstrated experience working effectively with the homeless and/or minority and underserved populations may be considered in lieu of academic degree.
Education: Bachelor’s degree in Social Work or related field.
Preferred Qualifications: Culturally sensitive to the social, psychological and health care needs of the homeless person and/or underserved populations. Demonstrated ability to work independently, as well as, with multidisciplinary teams and coworkers. Strong organizational, interpersonal and communication skills. Knowledge of HIV/AIDS, mental health and substance abuse issues.
Work Schedule: Ability to work a flexible schedule: Weekends, evenings and on-call, as needed.

MASTER LEVEL CLINICIAN
Reports to: Director of Programs
Summary: To save people who are homeless and suffer from medical psychiatric and/or substance abuse illnesses.
Responsibilities: Welcomes clients in a hospitable manner, in order to establish a positive connection. Conducts initial and ongoing screenings and assessments for persons seeking services and housing placement. Provides individual, group and family counseling. Participates in daily staffing, street and home visits. Coordinates services, refers and advocates for clients in the community. Other duties as assigned.
Requirements: Committed to the mission of Camillus House, Inc. Master’s degree in Social Work or related field required. License required: LCSW or LMHC. Valid FL-Driver-License. Ability to read and write English at the 12th grade level. Able to work flexible schedule. Strong interpersonal and communication skills. Demonstrated experience working effectively with the homeless and/or minority and underserved populations preferred.

MEDICAL SOCIAL WORKER
Reports to: Director of Direct Care
Purpose of Position: To provide case management services to medically involved clients in the Courtyard and Respite Bed Programs so that they receive the appropriate level of care.
Responsibilities: Welcomes clients in a hospitable manner, in order to establish a positive connection. Complete psychosocial assessments, provide case management and referrals for clients in the Respite Bed and Courtyard programs. Work collaboratively with the medical providers form Camillus Health Concern around the treatment plan and discharge planning issues. Serve as a liaison from Camillus House to Camillus Health Concern. Identify community services, alternative living arrangements and health related resources for clients who are homeless and medically needy. Work collaboratively with Camillus House case managers and attend supervisory and case management meetings. Serves as a resource to Camillus House’s and Health Concern’s staff regarding availability of resources. Maintains client records complying with fiscal, legal and professional standards of care. Assist with job related special projects as needed. Other duties as assigned.

Strong organizational skills. Strong written and oral communication skills. Computer literate. Demonstrated ability to read and write English at the 12th grade level. Valid FL driver’s license.

REGISTERED NURSE: ACT TEAM
Reports to: Psychiatrist
Responsibilities: Welcomes clients in a hospitable manner, in order to establish a positive connection. Maintains accurate detailed reports and records. The position is responsible for providing health care to patients as assigned by delegated staff (Psychiatrist or Program Manager). This includes the coordination of patient history and physical assessment, ordering laboratory work, diagnostic and treatment as delineated in Camillus Health Concern’s Medical policies and procedures.
Assist clients with medication compliance and education.
Assist Psychiatrist in making medical appointments and coordinating medical appointments with Camillus Health Concern. Attends and participates in ACT’s regularly scheduled meetings and other staff meetings as assigned by Psychiatrist and/or Program Manager. Attends required trainings.As an integral member of the ACT team, RN will be involved in tasks and duties necessary to achieve team’s goals
and objectives. Position requires ability to multi-task, work well with client population and be able to work well with team members. The position participates in ACT Team Model of Care, i.e. daily staffing, street and home visits as assigned by delegated staff. Coordinates services, refers and advocates for clients in the community.The position may be asked to perform other duties as assigned by the Psychiatrist.
Requirements: Committed to the mission of Camillus House, Inc. Nursing degree from an accredited University and professional license to practice field. Minimum two (2) years experience.
Valid FL-Driver-License. Ability to read and write English at the 12th grade level. Able to work flexible schedule. Strong interpersonal and communication skills. Demonstrated experience working effectively with the homeless and/or minority and underserved populations preferred.

RESIDENTIAL ASSISTANT (3 PER DIEM)
Reports to: Program Supervisor
Responsibilities: Welcomes clients in a hospitable manner, in order to establish a positive connection. Performs daily monitoring of all clients at the facility. Responsible for reading and maintaining daily logs. Responsible for assisting clients with daily activities. Monitors daily self-medication distribution. Ensures that facility is clean at all times. Assigns chores to clients and performs follow-up inspections. Ensures the dispensation and storage of supplies. Conducts room inspections and documents any maintenance/repairs needed. Responsible for distribution of food at meal times. Evening Shifts: Answers phones and takes messages when necessary. Informs supervisor of any problems that occur in the evening and early morning hours. Other duties as assigned.
Requirements: Committed to the mission of Camillus House in both knowledge and practice. Education: High school diploma or GED. Minimum 6 months previous experience in a social service setting.
Valid FL driver’s license. Demonstrated ability to read and write English at the 12th grade level.

SUPERVISOR OF COURTYARD PROGRAM/ SHELTER SERVICES
Reports to: DIRECTOR OF DIRECT CARE MINISTRY
Responsibilities: Welcomes clients in a hospitable manner, in order to establish a positive connection. Assists the Director of Direct Care Ministry in the coordination of an effective plan for the Courtyard and Shelter including equipment and supplies. Routinely inspects Courtyard, overnight Shelter and related service areas; e.g. shelters, laundry facilities, showers, toilet facilities, linen storage room, offices, reception areas, etc. Provides supervision to the Behavioral Health Technicians. Schedules Behavioral Health Technicians work schedules to ensure that all shifts are covered at all times. Assists the Director of Direct Care Ministry in maintaining all overnight shelter furnishings and reports repair and/or replacement needs to the Director of Facilities. Responsible for collecting and documenting client contributions. Assists the Director of Direct Care Ministry in keeping the overnight shelter free from possible sources of infection by insects and rodents. Reviews contract services with the Director of Facilities regarding pest control on a regular basis. Maintains an inventory of all shelter and janitorial supplies. Requisitions necessary cleaning and maintenance supplies and equipment from the Director of Direct Care Ministry. Contracts professional cleaning services for carpet maintenance, window cleaning, etc., as necessary and with the permission of the Director of Direct Care Ministry. Is responsible for the direction of all guest services in the Courtyard and Shelter Programs. Directs the proper handling, accounting, and issue of all bed linens, towels, etc. and will insure their routine laundering, repair and/or necessary replacement. Issues the distribution of shelter and housekeeping supplies as requisitioned by the volunteer staff. Assists in all scheduled ministry services in the facility. Conducts scheduled and unscheduled inspections of facility. Perform other job related tasks as directed by the Director of Direct Care Ministry.Other duties as assigned.
