Case Management

For many persons who are homeless, navigating the complicated web of social services and benefits programs can be a confusing, daunting, and even an impossible task. Case management services help clients develop individualized plans to address their situation, and then help guide clients through implementing those plans and accessing services. Case management services are an integral component of all Camillus’ housing programs. Services are offered in English, Spanish, and Haitian Creole.

Once screened, each client undergoes an extensive assessment and is assigned a Clinical Case Manager, who is responsible for the overall coordination of services for the client. Clients have access to on-site psychiatric services, specialized groups and day treatment programs utilized to promote life skills, coping skills, and therapeutic needs. Clients are also welcome to attend drop-in groups through the Day Center, offering open communication to increase awareness of services available. Camillus strives to eliminate all barriers to treatment, and is able to meet all of the individual's basic needs so clients are free to focus on their recovery. Housing, meals, clothing, and health care are all provided within Camillus' own continuum of services. Case Management clients may participate in treatment and services at Camillus, or may be referred and placed into other mental health programs, depending upon their individual needs. The Case Manager facilitates access to whatever the client needs, maximizing community resources.
