Adult Primary Health Services: Adult Primary Health Services encompass the full range of services, including preventive care, episodic care for acute illness, chronic disease management, and specialty and surgical referrals as needed. Preventative health measures are implemented with patients regularly screened for health problems particularly prevalent among the target population, including diabetes and hypertension. Health disparities are addressed through education, prevention, testing, and follow-up care. A full dispensary service provides medications at no cost for those who have no other resource.

Pediatric Health Services: Pediatric Health Services provides primary health care, including immunizations, health maintenance, disease prevention, and education. Many children seen at the health center and at the school-based program have preventable infections such as head lice, ringworm, and pinkeye as well as incomplete immunization status. Special attention is given to parent education and participation in the health outcomes of their children.

Women's Preventive Health Services: Women's Health Services focuses on health maintenance and prevention, providing cervical and breast cancer screenings. Women are educated on the importance prevention, including how to perform breast self-exams. Follow-up care and referrals for mammography are provided as needed.

Medical Outreach Services: CHC has its roots in medical outreach when in 1984 dedicated volunteer doctors took healthcare to homeless persons on the streets. In 2002, the program was expanded and now incorporates engagement, needs assessment, treatment and intervention, referrals, and transportation. Medical services are provided at sites frequented by homeless persons, including shelters and meal programs. Additionally, outreach is conducted by canvassing various street sites. Individuals needing additional health care services are referred to CHC's main site.
